What we hold ourselves to

A clinical record has to be defensible, not just tidy. In Holder, a finalised note is locked — any later change is an append-only amendment with a reason and an author, and the original never mutates. Consent is signature-grade and versioned: every response snapshots the form as it was, so a later edit never re-interprets an old answer. If a record is ever questioned, you can show exactly what was written, when, and by whom.
